On Friday 23 July fax this to Conference Managaers on 03 379 0460.
After Friday, bring this completed form with you to the Conference
Registration Desk at SKYCITY Convention Centre. 
Note that credit card payment in full or Purchase Order for invoice will be required for all late registrations.

Day registration $625.00 incl GST ($555.56 + GST)

•  Attendance to the day’s conference sessions
•  Satchel and contents
•  Morning and afternoon tea
•  Lunch
•  Monday registration includes Fulton Hogan Conference Dinner ticket
•  Additional ticket may be purchased for the social function not included in that day's registration fee

Partner registration

If you are bringing a partner to the conference, please register them using your own registration form if they wish to attend social or tour options.  Ensure that you complete their name details where requested.  All of these events have been priced separately so partners can choose what suits them.  Everyone registered as partners attending a social function/tour will receive a name badge.
 
If separate payment is required from the other registration costs please write this instruction on the manual form or for online registration once completed email registration@conference.co.nz at the time so this information is correctly captured.

Goods & Services Tax
GST Payable will be calculated at the rate of the day fees and tickets are invoiced.  The GST rate at the time of publication is 12.5%.

If you are selecting tours or social functions for your partner, please use the registration form to indicate their name.  If separate payment is required from the other registration costs please email registration@conference.co.nz at the time you complete your registration form so this information is correctly captured.

Cancellations
Should you need to cancel your registration, you may reassign your registration to another person. Please notify the Conference Secretariat in writing. If you are unable to arrange a replacement, a full refund less an administration charge of 25% of the registration fee providing notification is received by 26th June 2010.  No refunds will be made from 27 June 2010.  Please note that substitutes for all registrations will be accepted.  Delegates are not to share Registrations.

Charges for cancellations of accommodation, particularly within one month of the conference, will be subject to the discretion of the hotel concerned and your credit card may be charged a cancellation fee levied by the hotel.
